USE VT POLLING TO CREATE ONLINE FORMS!
A survey is nothing more than a list of questions, with free form or multiple choice answers. Now you can create a "survey" that includes all the questions on one of your forms. Allow residents to fill out the form and upon submission it will notify the person responsible for that form. They will be able to log into the site and download the data as a comma separated file (.CSV). If you already have another applictaion that will accept batch input you can integrated the data into your existing database. If you simply want a hard copy form, set up your form as a mail merge document and you can recreate the form with all the data fields filled out.
